About Our Office

Your Office Visit

At the time of your initial office visit, you will be seen by Dr. Kitchel, a physical therapist, and members of Dr. Kitchel's staff. Although this may prolong your initial visit, we are able to provide you with as thorough an evaluation as possible.

Your Patient Packet and Forms

Prior to your initial visit, you will be sent an introductory letter and packet of forms in the mail. These forms request information about you, your current medical condition, and your medical history. Please fill out these forms and return them to our office prior to your scheduled visit. This information is critical to our ability to assist you with your medical care. If you do not return your forms prior to your scheduled appointment, you may experience a delay in seeing the doctor.

Your Required Records

We can provide you with the very best medical care only when we are fully informed of your medical history and your current condition. It is important that all previous records, x-rays, and tests (e.g. MRIs, bone scans, CT scans, arthrograms, ultrasounds, etc.) relating to your current condition are available to the doctor at the time of your appointment. Please make sure that all records and results are forwarded to us. Please call two or three days before your appointment date to make sure that all of your records and test results have arrived at our office. If they have not, it may be necessary to reschedule your appointment to a later date.

Insurance Billing

As a courtesy, we are happy to bill your insurance, if you are insured. Please remember to bring your insurance card with you to your initial appointment, so that we can make a copy of your card and insurance information for our records.

Copayments

If you are covered by a managed care insurance plan, your copayment will need to be made at the time of your appointment.

Payment

For payment, Orthopedic Spine Associates accepts cash, checks, Visa, and Mastercard.
